| Super Choice Form | Managing your Super | A form you give to your employer asking them to pay your superannuation contributions into your Virgin Super account. |
| Letter of Compliance | Managing your Super | If you tell your employer you’ve chosen to make standard 'super contributions' to your Virgin Super account, give this to them so they know Virgin Super is a legally compliant super fund. |
| Personal Health Statement | Managing your Super | If you fit one of the descriptions below this is the form for you:

| Tax File Number Form | Managing your Super | If we don’t have your TFN on record we may have to charge you higher tax rates on your contributions. |
| Binding Death Nomination Form | Managing your Super | If you’d like to leave your super to a particular person/or people, we need to formalise this bit with your autograph. |
| Joint Financial Services Guide | About Virgin Super | An important document which we are required to give you. This FSG is intended to help you understand who we are, how we work, our relationship with the Administrator, Superpartners, and how we plan to work with you. |
| Regular Voluntary Contributions Via DD Form | Managing your Super | A form for setting up a direct debit for regular contributions to your Virgin Super account from your bank account. |
| Insurance Application Form | Managing your Super | For Death only or Death and Total Permanent Disability for a value up to and including $350,000. It’s where we keep all the little details. |
| Investment Option Form | Managing your Super | Fancy picking your own asset classes rather than a Life Stage Tracker option – this is the form you need. |
