Service forms

Form name Description
Standard Super Choice Form If you want your (new) employer to pay your standard super contributions to your Virgin Money Super account.
Letter of compliance Give this letter to your (new) employer along with the ATO choice form if you want your employer to pay you standard super contribution to your Virgin Money account.
Binding Death Nomination Form If you want greater certainty about who'll receive your benefit in the event of your death, you need to complete this form.
Change of Details Form If you need to update some of your personal details such as your name or an incorrect date of birth.
Trans-Tasman Transfer Form If you wish to transfer your Virgin Money Super to your KiwiSaver in New Zealand.
Baby Break Form Planning to go on maternity/paternity leave? Apply for your Baby Break benefit using this form.
Contributions Splitting Application Form If you wish to split your super contributions with your spouse.
Financial Hardship Form If you need to request your Virgin Money Super funds to be release on the grounds of severe financial hardship.
Salary Sacrifice Form If you wish to make a salary sacrifice contribution/voluntary contribution through your employer to your Virgin Money Super account.
Payment Instructions Form If you wish to apply for a super payout from your Virgin Money Super account. Funds can be withdrawn on compassionate grounds by completing and sending it in with the ATO approval letter.

Insurance forms

Form name Description
Tailored Insurance Application Form If you wish to apply for our Tailored Insurance cover for either Death & Total and Permanent Disablement (TPD) insurance or Death only cover for specific amounts and/or for Income Protection Insurance cover.
Insurance Cover Transfer Application Form If you wish to transfer your current insurance cover under another life insurance policy to Virgin Money Super.
Life Events Cover Increase Form If you wish to take up extra Death only or Death and TPD cover when a specific life event occurs without providing additional information about your health.
Reduce or cancel Tailored Insurance Form If you wish to reduce or cancel your Tailored Insurance cover for either Death & Total and Permanent (TPD) Insurance or Death only cover for specific amounts and/or for Income Protection Insurance cover
New customer insurance opt it As a new customer under the ‘Putting Members Interest First’ legislation, you are eligible for automatic insurance cover as part of your Virgin Money Super account when your balance reaches over $6,000 and/or you are 25 years and over. At the time, your insurance will be automatically turned on. If you would like insurance sooner as a customer you can opt in within 60 days of joining. Use this online form to get it in place.
